蕭偉志駕駛大貨車載運紙器從彰化南下，行經國道中山高麻豆路段因違規占用中間車道，國道警跟車約2公里蒐證後攔查開單，未料卻遭另輛大貨車從後面追撞當場喪命，家屬前往國道警四隊新營分隊製作筆錄時不願多談。
肇事的大貨車駕駛陸乙豪則在製作筆錄後表示相當抱歉，感到後悔，警方初步了解，陸男昨日下午約5點至位於路竹的公司載運牛乳，5點半出車前往桃園，今日0時10分自桃園出發返回高雄，1時30分在台中后里地磅站休息，3時繼續開車，4點34分行經國道中山高麻豆路段肇事，精神不濟是否因服用感冒藥等情形，仍待進一步查證。
